The Office of Home Care provides case management and outreach services to eligible individuals seeking to remain in the community rather than live in a long-term care facility through the use of specific home and community based services and informal supports. Adult Protective Services (APS)

The Adult Protective Services Unit (APS) is designed to investigate cases of abuse, neglect and exploitation.

Aging and Disabilities Resource Connection (ADRC)

The ADRC provides outreach, assessments and case management services to seniors and individuals with disabilities. Knowledge staff are available to provide information for such programs as adult day care, support groups, transportation, home care, respite, senior centers and home delivered meals to name a few. If necessary, a case mangers can complete a home visit and along with the resident and family identify the needs and create a service plan to meet those needs.

Jersey Assistance for Community Caregiving (JACC)

The Jersey Assistance for Community Caregiving (JACC) is a program that provides in-home services for seniors at risk of placement in a nursing home. JACC is designed to supplement the assistance and individual receives from his/her natural support network. Qualified individuals may have the opportunity to hire their own eligible family, friends, or neighbors to provide the care they need.

Outreach

Outreach staff conduct home visits to any Atlantic County resident 60 years of age or older in need of home delivered meals, personalized attention for completing applications or explaining programs and benefits for which the senior may qualify.

Personal Assistance Services Program (PASP)

The Personal Assistance Services Program (PASP) provides supports to adults with physical disabilities who are working, attending school or vocational training and living independently in the community.

Statewide Respite Care Program

The Statewide Respite Care Program (SWR) is designed to provide assistance to families by giving caregivers a break from the daily responsibility of caring for an elderly or disabled relative or friend.

The program provides respite care services to relieve caregivers of stress from providing daily care. This respite may be provided for a short time, or once in a while. Services can include adult day care, home care, companion services, campership, or a short stay in a facility.
